    Form to collect yearly statistics for early childhood ministries, Lutheran elementary and high schools, and preparatory schools.

Missions forms

  • Missions Promotional Materials Request

    Any congregation or Lutheran Women’s Missionary Society (LWMS) circuit is welcome to request up to 50 copies of Faces of Faith, our annual Missions magazine, 50 copies of our special 100 Missions in 10 Years brochure, and/or large home and world mission maps (24” x 36”) free of charge.

Special Ministries forms

  • Inmate Referral

    ​Form to refer an inmate for the ministry-by-mail program, which provides printed devotions, Bible studies, and Bible correspondence courses.

  • Military Referral

    Form to refer military personnel for the ministry-by-mail program, which provides Meditations, a devotional booklet, printed worship services, and e-mail support for those serving in the Armed Forces.

  • Mission for the Visually Impaired Referral

    People who are visually-impaired or print-impaired can receive materials via mail or an online Christian audio resource library called WELS Listen Library. This referral form allows qualified people (or their churches, family or friends) to connect with MVI for these free services.
